This presentation will highlight the collaborative work of an interdisciplinary palliative care team within a clinic setting as they navigate the complexities of challenging patient care cases. Through this case study presentation, the audience will explore how physicians, nurses, social workers, and other community partners contribute to the unique expertise to address the patient's and support's needs. The presentation will emphasize the importance of communication, informed decision-making, coordinated care planning for support.
