Title: Breaking Barriers: Tools and Resources to Improve End-of-Life Conversations Description: Conversations about death and dying are often avoided, even in hospice and palliative care settings, yet they are essential for aligning care with patient values, reducing conflict, and supporting families during some of life’s most difficult moments. This session introduces practical and creative approaches to help professionals foster more effective and compassionate end-of-life discussions. Drawing on nearly two decades of hospice and emergency medicine experience, as well as creating innovative communication tools such as The Death Deck, The E•O•L Deck, and The Dementia Deck, I will demonstrate how structured resources can transform intimidating conversations into approachable, engaging, and meaningful exchanges. Participants will explore the role of guided questions, storytelling, and even game-based interactions in promoting dialogue that empowers patients, caregivers, and care teams. Through interactive demonstrations and case examples, attendees will have the opportunity to practice using conversation prompts and reflect on their own comfort levels with end-of-life communication. Emphasis will be placed on strategies that professionals can immediately apply in both clinical and community settings to normalize these vital conversations and enhance patient-centered care.
Learning Outcomes
By the end of the session, participants will be able to: Identify three common barriers that patients and families face when discussing end-of-life preferences. Describe at least two structured tools for facilitating these conversations. Identify at least one strategy for enhancing their own communication practice. This session equips hospice and palliative care professionals with practical, innovative tools to strengthen communication, reduce family conflict, and promote peace at the end of life.
