Deprescribing is a fundamental task of the HPM provider, and presents clinical as well as psychological and emotional challenges for patients and care teams. We will use a case-based format to illustrate concepts of rational and safe deprescribing in patients with serious illness.

By the end of the session, learners will be able to:

1. Identify medications and combinations that could potentially cause harm to a seriously ill individual.
2. Rationally reduce the burden of unnecessary medications in a safe manner.
3. Guide patients and families through the process of deprescribing.
