This session reviews the outcomes data and risk/benefit profile of anticoagulant/antithrombotic medications to help hospice staff and clinicians make informed, data-driven decisions about continuing or deprescribing these medications in hospice and palliative care.

  • Understand the annualized VTE and stroke risk of various conditions based on clinical research.
  • Understand the data on bleeding risk for hospice/palliative care patients.
