Lori Braegelmann is Program Manager, Advance Care Planning at CentraCare. Lori enjoys building visibility and momentum surrounding ACP with CentraCare’s internal team, patients, and the external community. She is a Certified ACP Facilitator and Certified ACP Instructor. Lori’s previous background lies in Community Engagement and Program Management for a chronic health condition and is accustomed to building momentum surrounding health topics that carry some misunderstanding and stigma. She worked 14 years total for Epilepsy Foundation of MN (covering 21 counties in Central and West Central MN) and the Epilepsy Foundation of America (building out chapters in 18 states). Bringing Advance Care Planning out of the shadows is her current mission and she loves building relationships and driving strategies that support ACP.
