This session will provide the learner an overview of  Advance Care Planning and the positive impacts it makes on patients, families and systems.  

Upon completion of this educational activity, attendees will be able to:

• Define Advance Care Planning (ACP) 
• Identify positive outcomes for patients, families and systems when ACP is done
• Identify when patients should have an Advance Directive completed and when to update them
• How ACP can help patients and families in their decision making
