Jenny Sanderfoot, MSW, CSW, is the Advance Care Planning Specialist for ThedaCare. Jenny’s been a social worker for over 22 years, with 10 of those being in healthcare including: hospital care management, nursing home, hospice and outpatient/complex care management. She’s a certified Respecting Choices Facilitator and Instructor.
She been an advocate of Advance Care Planning after seeing the stress and heartbreak that results when end-of-life wishes aren’t discussed with loved ones in advance. As the ACP Specialist for ThedaCare, Jenny’s role entails creating, implementing and rolling out work flows, education and resources for all staff and patients. Her work also includes collaboration with community partners such as the Fox Valley Advance Care Planning Partnership (FVACPP).
