Description:  The goal of the MNHPC 2025 Conference is to advance the knowledge of healthcare professionals who provide serious illness and end-of-life services. This conference is designed for nursing, administration, music therapy, physical therapy, chaplaincy, medicine, occupational therapy, social work, physician assistants, home health care, and volunteers. 

This activity has been planned and implemented in accordance with the accreditation requirements and polices of the Accreditation Council for Continuing Medical Education through the joint providership of the Minnesota Medical Association and MNHPC. The Minnesota Medical Association (MMA) is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. In addition, this educational activity has been planned and implemented to meet accreditation requirements and standards in accordance with the entities and disciplines listed below. Learners should consult the appropriate disciplinary area below for information about continuing education contact hours for this event.  In order to earn contact hours for this event, regardless of discipline, conference attendees must attend one or more live sessions in their entirety and complete a post-event evaluation.


Learning Outcomes:  At the conclusion of this activity, participants will be able to:

  1. Summarize best practices and innovations in serious illness and end-of-life care.
  2. Describe specific pain and symptom management interventions for patients receiving hospice and/or palliative care.
  3. Understand common emotional and spiritual needs of patients, families, and professionals caring for patients with serious illness and end-of-life.
  4. Describe strategies and practices to address complex family dynamics, grief, bereavement, and trauma at the end-of-life.
  5. Identify ethical challenges related to caring for patients with serious illness or at the end-of-life.
  6. Summarize hospice and palliative care regulations, policies, and reimbursement strategies and identify strategies for compliance.
  7. Identify strategies to raise awareness of hospice and palliative care across all Minnesota communities.

 

Earning Contact Hours
To receive a certificate of completion for credits earned during the MNHPC 2025 Conference, attendees must:
Fully attend each session of the live event for which the attendee is seeking credit. 

  • Complete the post-conference evaluation. A link to the evaluation will be emailed to attendees within 5 business days following the conclusion of the conference.  
  • If seeking MOC points, additional information regarding DOB and license # must be provided to MNHPC. Contact info@mnhpc.org for more information.  
  • Certificates of completion will be sent via email.

 

Medical/ACCME

Minnesota Medical Association
The Minnesota Medical Association designates this activity for a maximum of 11 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.


This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the Minnesota Medical Association and MNHPC. The Minnesota Medical Association (MMA) is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. 
 
 

American Board of Internal Medicine
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 11 medical knowledge MOC points. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
 

American Board of Pediatrics
Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn up to 11.5 MOC points in the American Board of Pediatrics’ (ABP) Maintenance of Certification (MOC) program. It is the CME activity provider’s responsibility to submit learner completion information to ACCME for the purpose of granting ABP MOC credit.  
 

Nursing
This activity has been approved by the Wisconsin Nurses Association for approval of 11 continuing education (CE) credits. The Wisconsin Nurses Association is an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.  Please note that not all sessions have been approved for nursing credit
 

Long Term Care Administration
This professional development activity has been approved to meet the Minnesota Board of Executives for Long Term Services and Supports requirements for 11 contact hours (Approval Code MN24-00094).
 

Music Therapy
This professional development activity has been approved by the Certification
Board for Music Therapists (CBMT) for 11 music therapy continuing education credits*. Minnesota Network of Hospice & Palliative Care, #P-153 maintains responsibility for the program quality and adherence to CBMT policies and criteria. 

*Those seeking music therapy credit are reminded that a CMTE credit is equal to one 50-minute hour of direct coursework or contact. Contact hours are calculated from clock hours (60 minutes). CMTE credits are not granted in units smaller than 1 credit; fractions of hours are rounded down (e.g. 3.5 contact hours are recorded as 3 CMTEs).

Social Work
This workshop has been designed to meet the Minnesota Board of Social Work requirements for Continuing Education activities as designated in Minnesota Statutes section 148E.145 and is eligible for 11 clock hours.
